This legal document describes the types of medical care you want and doesn’t want to receive (including end-of-life activities) and who can speak for you if you are unable to make medical decisions for yourself. Give yourself and your family peace of mind by getting it done!
Caseworkers from Hospice of Santa Barbara will be at the Santa Barbara Museum of Natural History to help you complete your plan. A notary will be onsite in the Museum’s Fleischman Auditorium providing free notary services to help you finalize it. Hospice staff will shepherd your plan to Cottage Health, so it’s on file and ready when needed.
