Advance care planning helps identify, communicate, and document the health care choices you would make if you could not speak for yourself in a health crisis. Having these discussions in advance and documenting them in an advance directive helps adults of all ages to prepare for unexpected situations so medical providers and loved ones can respect and honor their health care wishes.
An advance care planning facilitator will explain the various healthcare decisions to consider including how to choose a healthcare agent, and answer your questions so you can complete your MyCare advance directive.
